I been try to refer from the article and set the option 3: If from a smartphone, show a mobile-friendly callout for the Mobile Optimized Form (RECOMMENDED)
But the tab still hide it up and show the tiny bar only on me and customer's iphone.

any solutions to fix it out? really urgent need to get ready for customer.
